Bitten by an insect on an away mission, Janeway and Chakotay contract a virus the Doctor cannot cure. Only the planet's environment holds it in check, so they must stay. Janeway orders Tuvok to take Voyager home without them, and forbids him to contact the Vidiians, who might know of a cure but cannot be trusted. On the planet Janeway keeps searching for a treatment until a storm wrecks her equipment, while Chakotay turns their camp into a home, and the two grow closer. Aboard Voyager morale collapses, and Kim openly challenges Tuvok's refusal to approach the Vidiians. Persuaded by Kes, Tuvok relents and reaches Dr Denara Pel. The Vidiians spring a trap, but Pel secretly sends the serum and Tuvok outwits the attackers. Voyager turns back, and Janeway and Chakotay leave their new life behind to return to their posts.
